Is $3,400 to jack up shed and pour concrete slab a fair price?

I recently bought this home. The 10 foot wide by 16 foot long shed in the backyard is shifting off its cinder blocks because they are sinking. The shed is in decent shape and appears to be less than 5 years old. I'm considering this concrete slab option as demolishing and rebuilding would be more expensive, but I'm open to other repair ideas.
